PATIENT INFORMATION AND DOCUMENT RESOURCE PAGE
The Davenport Clinic has provided access to medical forms and documents for our patients use. Please complete these forms and bring them with you to your appointment. It is important to know your prescription medications, over the counter medications and herbal supplements. Remember to provide any past medical, surgical, and family histories as they are also an important part of your medical chart.

CANCELLATIONS:
|
If you need to cancel your appointment, please call 24 hours in advance. We understand that occasionally you may not be able to contact us in advance. If this becomes habitual, you may be dismissed from the clinic.

CO-PAYS:
|
Your co-pay is required at the time of service. We accept cash, check, Visa, or MasterCard.
|
|
MISSED APPOINTMENTS:
|
1st Missed appointment; you will receive a letter in the mail.
2nd Missed appointment; you will be charged a $25 "missed appointment" fee.
3rd Missed appointment; you will be charged in $25 "missed appointment" fee and you may be dismissed from The Davenport Clinic.

PRESCRIPTIONS:
|
Please give us 24-HOUR notice on all prescription refills. Check with your pharmacy the next day to see if your prescription is ready BEFORE you call the Davenport Clinic.

NARCOTICS/PAIN MEDICATIONS:
|
If the doctor prescribes pain medication and/or narcotics, you may be required to pick up this prescription at the Davenport Clinic. Most of these prescriptions will now require an office visit every (3) months and you will be given this prescription at the time of your office visit.

|
SELF-PAY:
|
If you do not have health insurance, we will be happy to see you. Please bring a minimum $50.00 to your first appointment. After your first appointment, payment arrangements will be made. If you do not have your minimum payment when you come to your appointment, you will be rescheduled.

ON-LOCATION SERVICE
We have an X-Ray facility on site.
We also offer Lab work on site in agreement with Mississippi Valley Lab.

PHARMACIES
Davenport Clinic can e-scribe to most of the pharmacies in this area and we will work with any pharmacy you choose.
